Form 1095-C for an eligible employee may be requested from the company by emailing hr.support@gnyservices.com, call (201) 217-0707 or requesting a copy in person at our main address of 26 Journal Square Plaza #305, Jersey City, NJ 07306. It shall be furnished to the employee within 30 days of the request.
