FAMILY NURSE

ACCESS Nursing Services will handle patient care upon admittance into the hospital, once you’re back at home and onto the moment that you complete outpatient care. Transitional care prevents a gap in care between the hospital and home, and ACCESS Nursing Services makes it easier for patients and their families to find the right course of transitional care. Patients receive a free care management consultation with an ACCESS Nursing Services certified nurse practitioner when they enter the hospital. Our nurses coordinate a care management plan with each patient and the hospital’s provider team to ensure the health plan fits both the patient’s medical needs and their lifestyle.

Post-discharge patients and their families can feel secure in a continuity of care that fits seamlessly into the lifestyles of patients and their families. Nurse and healthcare professionals transport the post-discharge patients into their homes, ensuring that their ride is both safe and comfortable. Once the patient reaches their home, the nurse discusses the patient’s healthcare instructions with the family and educates family members about medication management and daily care.

ACCESS Nursing Services staff serves as a liaison between patients and their physicians, as well as with healthcare insurance companies. Staff will also contact patients and their families to ensure and maintain a high level of patient satisfaction during the beginning, middle, and end of the care plan.

AMBULATORY CARE

Every patient travels comfortably from the hospital to the home when they have the assistance of an experienced nurse or medical professional who always puts patients’ health and safety first. Medical professionals will help patients settle into their homes, accommodate any personal or medical needs, such as changing catheters or picking up medications. Staff specialized in elder, adult and mother/infant care are available with the option to extend outpatient services. Services include transportation in a handicap-accessible vehicle from hospital or medical center to home. Only 24 hour notice is necessary.
